About the Role You will deliver high-quality, person-centred supports in participants’ homes and within the community. Key responsibilities include: Supporting community access and social participation Providing personal care and daily living assistance Delivering capacity building and skill development supports Providing behavioural and psychosocial support where required Maintaining accurate documentation, case notes, and progress reporting This role requires reliability, professionalism, and the ability to work both independently and collaboratively within a multidisciplinary environment. About You We are seeking compassionate and motivated individuals who are committed to empowering participants and promoting choice and control.
